The act of swallowing saliva and clearing the throat is a reflex action that’s done in each of us, but death rattle occurs when the person is unable to do this or even swallow phlegm from the back of the throat. They lack the strength to do it, their breathing pattern might change and breaths might vary between quiet to loud. Sometimes they breathe in sharply and then skip a few moments of breathing and then breathe again.
